Essential Functions & Responsibilities: The RN works under the direction of Physician or Advanced Practice Provider.
1.Assesses health care needs of individuals and provides telephone and face-to-face triage to assess changes in condition based on standards of practice; assists patients with obtaining access to care based on patient need and request for care; responds to calls from patients and uses data to determine medical nursing needs and risk status.
2.Provides customer service, patient-centered care through telephone and face-to-face communications to assess changes in condition using established protocols and medical consultation about action plans as needed; responds to calls from patients, pharmacies, homecare agencies and family members; collaborates with community agencies and members of the interdisciplinary team, office staff, and other Triage Nurses.
3. Provides oversight of MAs and LPNs in absence of RN Clinical Lead /DON.
4. Must be able to organize day’s activities to keep a steady flow of patients through the clinical area.
5.Provide clinical direction for disease specific programs.
6.Perform standard laboratory examinations Hgb, blood glucose, urinalysis, ect and the corresponding quality control testing and log maintenance.
7. Administers ordered medications and immunizations as ordered by the provider.
7.Provide diagnostic therapeutic treatments as indicated such as EKGs, respiratory treatments, etc.
8.Accurate charting and proficiency using ECW and EPIC.
9. Assists provider with procedures and serves as chaperone when appropriate.
10. Participates as part of the Emergency Response Team during Rapid Responses or other emergencies.
11. Oversees regulatory readiness including but not limited to quality control testing of point of care equipment, equipment safety checks, and quality rounds.
12. Receives training and is designated site administrator for the Vaccines for Children Program through the State of Connecticut.
11.Abides by organizational policies and procedures.
12. Provides independent nursing visits within scope of practice.
13. Participates in Pre-Visit Planning for assigned provider’s patients.
14. Identifies gaps in care related to annual screening exams and schedules patients for screenings as per standing orders/protocols.
14. Attends pre-clinic huddle with provider to plan for day.
15. Rooms patients for assigned provider including taking vitals, medication reconciliation, etc.
Active RN license and CPR certificate required.
