The Program Coordinator is responsible for the coordination of the Center for Key Populations (CKP) HRSA Funded QIF – TJI grant Program – “Women in Transition” (WIT). The Program Coordinator is the key liaison between the Department of Corrections (DOC) staff, potential patients, communities who receive services from the program, community collaborators, and community partner organizations, as well as CHCI staff; The PC will oversee development and implementation of the overall program, ensuring widespread dissemination of information and efficient enrollment, engagement, retention, and follow up for program participants by staff. The PC will work closely with the Director of CKP and the two CHW staff to complete data analysis, create methods of dissemination, and to use data and evaluation information to improve the quality of the program. The PC will also work closely with the CKP Evalaution Coordinator to design a Program Evaluation including all deliverables and outcome measurement.
ROLE AND RESPONSIBILITIES
- Develop policies, procedures, manuals, and training as needed for specific workflows of the QIF – TJI grant program.
- Ensure compliance with all safety and regulatory requirements in accordance with the MOU with York Correctional Institute.
- Complete and monitor data entry, record keeping, and reporting for staff and patients.
- Conduct and maintain community outreach and collaboration with community organizations and partnerships and ensure effective coordination of all community referrals along with staff.
- Work with WIT staff to coordinate schedule of hours and services and arrange scheduling and work with DOC liaisons to provide services to as many participants as possible.
- Manage patient enrollment, engagement, retention and participation in follow up.
- Establish and maintain a community resource directory
- Assist with the facilitatation of regular meetings with staff and CKP leadership to ensure effective communication
- Supervise CHW staff in their work with soon to be or recently released individuals and provide all necessary support for staff.
- Assit to train staff at CHCI about the WIT program and how patients can obtain needed services.
- Maintain comprehensive records for staff training including professional development opportunities and updated program information.
- Coordiante on-going evaluation meetings and facilate participation of all WIT staff in the design and implementation of a comprehensive evaluation plan and deliverables.
- Plan and implement community events to promote services of WIT Program.
- Work in collaboration with CHC staff to identify sustainability plans for WIT Program while also working toward on-going future funding.
- Performs other related duties as assigned.

PHYSICAL REQUIREMENTS / WORK ENVIRONMENT
This position requires some physical exertion, mostly in support of groups in a program area.
Work is both remote, office-based and community-based
WORK SCHEDULE DEMANDS
Full-Time, 40 hours a week with evenings and / or weekends required based on program needs. Ability to travel to locations as deemed necessary
