About the Role:
We are seeking an experienced Outreach Worker to join our team at the Behavioral Health Center. As an Outreach Worker, you will be responsible for providing support and assistance to individuals with mental health and substance abuse disorders. Your primary goal will be to connect clients with the appropriate resources and services to help them achieve their recovery goals. You will work closely with a team of healthcare professionals to ensure that clients receive the highest quality care.
Minimum Qualifications:
- Bachelor's degree in social work, psychology, or a related field
- Experience working with individuals with mental health and substance abuse disorders
- Knowledge of community resources and services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Preferred Qualifications:
- Master's degree in social work, psychology, or a related field
- Experience working in a community-based setting
- Bilingual in English and Spanish
- Certification as a Certified Alcohol and Drug Counselor (CADC)
Responsibilities:
- Conduct outreach activities to identify and engage individuals with mental health and substance abuse disorders
- Assess clients' needs and develop individualized care plans
- Provide support and assistance to clients in accessing community resources and services
- Collaborate with healthcare professionals to ensure that clients receive comprehensive care
- Maintain accurate and up-to-date records of client interactions and progress
Skills:
As an Outreach Worker, you will use your excellent communication and interpersonal skills to build relationships with clients and connect them with the appropriate resources and services. You will also use your knowledge of community resources and services to provide support and assistance to clients.
