POSITION SUMMARY
Community Health and Wellness Partners (CHWP) is an integrated primary-care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. The Outreach Associate is the first point of contact for all patients utilizing our facilities for health care; manages the timely and accurate collection of important patient demographic information, and functions as a resource to patients entering the facility. Supports and acts as an advocate for patients by assisting them with the registration process. Conducts outreach efforts and implements financial assistance programs of CHWP. Supports team-based care model by focusing on the organizations Mission, Vision and Values.
PRIMARY ACCOUNTABILITIES

- Manage/assist with all patient registration and administrative processes as assigned. Ensure all relevant patient registration records collected are accurate, current, and completed in a timely and accurate manner.
- Ensure all clients/patients served experience a high level of customer satisfaction and care.
- Provide support for additional administrative and support activities within the registration/business office, maintaining an acceptable level of productivity and consistency with all tasks as assigned.
- Promote and exemplify excellent customer service to ensure that all clients/patients are served with a high level of customer satisfaction and care.
Operational Excellence
- Ensure that all records collected, processed, entered and filed are done in a timely, accurate manner.
- Ensure and uphold the confidentially requirements of all patient records and manage all daily tasks and activities consistent with HIPAA, state and federal laws and regulations, as well as the clinics policies and regulations regarding confidentiality and security.
Relationship Management
- Develop and ensure effective, positive relationships within and among all team members, as well as with patients, vendors, contractors, and related resources.

- Volunteer to serve the community by offering five (5) hours annually to a community service program.
PRIMARY TASKS & DUTIES
- Performs bed-side registration, patient intake, and rooming.
- Performs various clerical duties within the Team-care model. Prepares and maintains medical records as well as all intake/patient information.
- Schedules visits, tests and procedures as directed. Prepares requisitions for testing.
- Utilizes computers, adding machines, fax machines, phones, and other related office equipment in the fulfillment of assigned duties.
- Communicates verbally and/or by writing with staff members, patients, and their families.
- Answers patient inquiries about their account information, insurance filing, co-pays, etc. or directs callers to appropriate department. Takes patient co-pays.
- Ensures all paperwork required for the registration process is prepared accurately and timely.
- Maintains accurate, thorough filing of all paperwork.
- Participates in meetings and training for professional growth.
- Supports other CHWP activities and performs other duties as assigned by Clerical Outreach Coordinator.
- Supports Clerical Outreach Coordinator in conducting any and all patient care correspondence.
- Supports Team-care model when working with all CHWP staff.
- Performs other activities and duties as assigned by Leadership, Management or Administration.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES
- Function in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail.
- Adheres to evidence-based practices such as patient safety, infection control and clinical quality.
- Communicate with a diverse, sometimes difficult, patient/customer population. Will be required to successfully manage patients and families in often stressful or conflict situations.
- Utilize a wide variety of office technology and tools to collect, enter, process, file patient records and documentation.
- Any and all other duties as assigned by Leadership, Management or Administration.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.
WORK ENVIRONMENT
The work environment characteristics are those for a normal community health center environment (ambulatory primary care office). Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may require occasional weekend and/or evening hours.
Work hours
Full-time, non-exempt position. Generally, a 40-hour work week is required as a minimum; hours are subject to change. Office hours vary by site, but are usually 8:00am to 7:00pm, Monday through Friday. Some Saturdays and after business hours work may be required. Assigned meetings and travel are a part of the position. PRN (As Needed) may be available
Wages
Starting wages for the Outreach Associate will be negotiable based on Community Health & Wellness Partners pay policies and factors such as education and experience.
Introductory Period
Ninety (90) calendar days probationary period. The successful completion of the introductory period does not alter the at-will employment status.

QUALIFICATIONS AND REQUIREMENTS
Education
- High School diploma or equivalent.
- Must be able to speak read, write and understand English.
- Bilingual in Spanish helpful.
- Outreach/Eligibility training as required.
- BLS Certified.
Certificates and Licenses
- Valid Ohio driver's license.
Professional Experience
- Demonstrates skilled business office experience.
- Demonstrates success in communication, customer service or working with the general public, preferably in a medical care facility.
- Demonstrates success in managing difficult customer situations; sound judgment and critical thinking/problem-solving skills.
- Demonstrates success in general computer competence including basic Word and potential to be trained on specific software for patient information, billing and communication.
- Knowledge of medical terminology preferred.
Physical/Environmental
- Ability to interact with computer screen for up to six hours at a time (visual acuity required).
- Must have manual dexterity for use of keyboard. Ability to remain stationary for periods of up to four hours. Ability to communicate via phone, mail and in person to resolve disputes, solve problems, etc.
- Capacity to function in a sometimes stressful, multi-tasking environment.
