What are the responsibilities and job description for the Community Marketing Liaison position at Community Healthcare of Texas?
Markets the hospice program to physicians, discharge planners, and other referral sources; serves as a liaison between hospice and the medical community.
We are searching for marketing professionals to cover the following territories: 1) HEB/Grapevine/Keller 2) Denton/Flower Mound/Irving/West Dallas
Essential Functions
- Responsible for generating appropriate referrals from a variety of sources by meeting with physicians, hospital discharge planners, and other hospital staff to promote hospice.
- Maintains community visibility; attends networking programs.
- Facilitates and coordinates education programs and opportunities in hospitals, physicians' offices, and community settings.
- Track and report monthly contacts with referral sources and submit to supervisor.
- Participate in weekly team conferences contributing necessary information and documentation assuring the best quality of care to each patient.
- Assist with special events, health fairs, public relations as needed.
- Assist with the admission process by meeting with the patient and/or responsible party and completing documentation as needed or assigned.
Qualifications
- College degree or equivalent experience
- 2 or more years of marketing experience
- Hospice or Home Health experience
- Proficient skills using a computer
- Must be able to build relationships and communicate clearly
