WHAT WE ARE LOOKING FOR:
Seeking a professional member for our healthcare team! You will work directly with the supervisor and management of Community Healthcare Partners LLC as well as the providers and staff to ensure patient care needs are met. Maintain and monitor patients health status, participate in patient CCM (Chronic Care Management) . Work with providers and office personnel to ensure proper care plans are carried out for each patient. Make appropriate communications as needed to providers, physicians, pharmacies, supply companies, home healthcare co, and etc. Provide direction to patients when new cases / skills/results/medications/orders from the providers involved. Provides patient education and ensures all patients have what they need in order to treat their chronic illnesses as well as respond to patient acute needs.
Duties
- Maintain accurate records and documentation related to patient care and clinic operations. Perform initial patient record review to establish patient care with CHP.
- Be the point of contact and CCM for assigned patients.
- Drive Patient education
- Communicate needs to provider as they arise with patients
- Assist providers in coordination of care with specialists/home health/pharmacies, supply companies etc. All efforts are documented accurately.
- Work with office personnel to ensure proper care plans are carried out for each patient including schedulers, medical records, billing, coding, etc
- Other duties as assigned
