Job scope:
The Care Team Manager develops and implements a
comprehensive program for clinical health services, including standard of practice
in medicine, nursing, lab and behavioral health. The Care Team Manager directs the clinic
operation, delivery, oversight and evaluation of all patient care services
provided in the Primary Care clinics.
The Care Team Manager leads the medical providers, nursing staff,
behavioral health service providers, and administrative staff to work as a high
functioning integrated team. The Care
Team Manager participates in planning, implementing and evaluating the clinical
operational activities to maximize patient outcomes as well as meet
programmatic metrics. The Care Team
Manager applies knowledge of healthcare goals and objectives and the
organizational business plan to bridge patient care services and outcomes. Assists with the development and preparation
of the budget, monitors and authorizes expenditures. Initiates planned change, evaluates outcomes
and implements corrective action through conscious, deliberate and
collaborative efforts.
Education/licensure:
Graduate of an accredited school in nursing, business,
counseling, social work or other related health care field, professional
license preferred. Current valid driver's license in state of residence. Proof
of automobile liability insurance and insurable by the Center insurance carrier. License preferred.
Experience/Knowledge:
Possess a minimum of three to five years'
experience. Demonstrated or prepared to
provide effective management skills in an integrated health care setting where
primary care and behavioral health care are provided. Staff supervision, computer software
programs, budgetary work, and state/federal rules and regulations pertaining to
the operation of the program.
Physical requirements that are bona fide occupational
qualifications essential to the performance of the job: Ability and
strength to sit, bend, and reach during an 8 to 10 hour day. Able to lift a minimum of 10 pounds. Sensory acuity sufficient to evaluate, treat,
and monitor patient needs and maintain accurate records. Fine motor skills sufficient to legibly
document and communicate.
