Job Summary
Licensed Social Worker will perform psycho-social assessments as well as, support patients and family with community resources and discharge planning. Social Workers cannot perform medical assessments, discharge planning assessment or facilitate the high risk tool.
Essential Functions
- Performs discharge planning tasks.
- Maintains Patient confidence and protests patient personal health information (PHI).
- Connects patients/families into community resources.
- Interviews and supports patients with post acute care needs to meet optimal health goals via psych-social evaluation.
- complies with all federal, state and local legal requirements of their license and scope of practice.
- Contributes to interdisciplinary team meetings.
- Maintains operations by following policies and procedures.
- Participates in Process Improvements.
- Performs other duties as assigned.
- Complies with all policies and standards.
Qualifications
- Bachelor's Degree Licensed Bachelor degree Social worker required
- Master's Degree Licensed Master's degree social worker preferred
- 2-4 years Acute care hospital setting required
Knowledge, Skills and Abilities
- Strong written and verbal communication skills
- Knowledge of patient rights and laws relative to those rights, such as HIPAA
- Excellent organizational and multitasking skills.
- Ability to forecast for the near future (six to twelve months) is required
- Provide short-term case consultations and ongoing care coordination services to older adults and adults with disabilities.
- Ability to plan, organize and direct the activities of others.
- Knowledge of family systems and the ability to provide a supportive environment.
Licenses and Certifications
- LMSW - Licensed Medical Social Worker - State Licensure Licnesure and level of SW based on state requirement required
