Community Horizons assists with the recruitment process of a care partner to assist a member of the community with social engagement and enjoyable companionship activities. Have you explored Self-Directed Services where you will find Less Stress-No Forced shifts-working 1:1 in the individual’s home?
Schedule: Saturdays 5 to 6 hours (Minimum of one Saturday a month)
Wage: $15 per hour plus mileage reimbursement

Ideal Candidate:
· Kind and Compassionate
· Patient
· Experience working with Autism

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
