What are the responsibilities and job description for the Home Health Caregiver AM/PM Flexible position at Community Horizons LLC?
Community Horizons is excited to be helping a neighbor in need and is looking for your assistance! This job is in a private home on the east side of Green Bay. You would be hired directly by the individual to provide one-on-one care. Spanish-speaking caregivers are welcome to apply! Bonus points if you can cook a Mexican-style meal.
Work Hours Available:
Monday-Friday 1PM-3PM
Monday-Sunday 6PM-8PM
Saturday-Sunday 10AM-12PM
Willing to work with hours you can provide!
Pay Rate: $16.32 per hour
Your Responsibilities: Help with dressing and moving (using a Hoyer)- Assist with bathroom needs and handling a catheter bag- Help open medicine bottles- Do some light cleaning around the house
Who We’re Looking For:
Someone reliable who communicates well about any schedule changes- Must have a valid driver's license and a dependable car.
Must pass a caregiver background check in Wisconsin
Previous experience in caregiving is a plus, but we are happy to train the right person
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
