Schedule:
Monday-Friday from 8 AM to 2 PM
What You'll Do:
- Assist with personal care, like bathing, toileting, and dressing
- Help with transfers using a stand/pivot method
- Support with meal prep as needed
- Assist with repositioning
- Take care of light housekeeping tasks
- Lead range of motion exercises
What We’re Looking For:
- At least 3 years of caregiving experience
- A reliable attitude and a strong work ethic
- Ability to follow directions easily
- A valid driver's license

Community Horizons provides support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) across Wisconsin. Self-direction allows participants to choose who provides their services and how they receive them. Community Horizons does not directly employ caregivers; instead, members make all hiring decisions with the assistance of a third-party fiscal agent responsible for payroll.
