Community Horizons is assisting in the recruitment process for a personal caregiver to provide top-notch care to a fellow community member. This individual is seeking a caring, reliable and passionate caregiver to help maintain an independent lifestyle. Discover what 1:1, self-directed care is all about!

Schedule: 3 days a week (1-to-1.5-hour shift)

Wage: $16.50 per hour

Responsibilities:

· Shower assistance

· Transfer assistance utilizing a slide board

· Cleaning bathroom

· Assistance with dressing

· Wound care assistance as needed

· Other personal care tasks as needed

Ideal Candidate:

· Female

· Must be able to pass background check

· Caregiving experience

· Good Sense of humor

· Positive Attitude

· Solid work ethic

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
