If you love taking leisurely walks, curling up with a good book, or getting creative with arts and crafts, this could be the ideal opportunity for you. In this role, you'll assist with tasks such as using a specialized lift, managing medications, supporting tube feedings, and ensuring a joyful and engaging experience every day!
Community Horizons is passionately supporting the search for a private caregiver to join a loving in-home care team. In this fulfilling role, you will provide vital support and hands-on assistance with personal care and housekeeping tasks. Your efforts will play a crucial role in helping this community member maintain their cherished independence at home.
This position is part of the self-direction program, which means you'll work directly for the individuals who need your support. This arrangement not only offers greater flexibility and control over your schedule but also fosters a more personal and meaningful working relationship, as the member will hire you directly, bypassing the agency.
Schedule:
Position 1: Monday 4 – 8 pm, every other Saturday 9 am – 1 pm
Position 2: Back up Mon – Thur 4 – 8 pm, Saturday 9am – 1 pm
Pay: $16.32 / hour
Tasks:
- Tube Feedings – no previous experience required, we’re happy to provide training
- Helping out with hygiene and dressing to keep things comfortable
- Using the Hoyer lift for safe and easy transfers
- Getting involved in fun activities like books, music, crafts, and more to brighten the day
What it takes to succeed:
- Must be 18
- Have a vibrant and fun personality
- Background must be favorable

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
