Hello! Community Horizons has partner with a vibrant 38-year-old individual born with Spinal Muscular Atrophy (SMA type 2) in her recruiting journey!
She is looking for a part to full-time caregiver for various mornings during the week, some daytime hours during the week, and some overnights during each month. She has been self-directing her own care for all of her adult life!
Her caregivers wear many hats, as part of her care is very medical-focused (urostomy, use a hoyer lift, a shower chair, physical therapy, etc), while the other part of the cares include helping to live a fulfilling life, as she participates in work, hobbies, and adventures!
This individual is very active with her wheelchair -- She has a love for hiking accessible trails and participating in adaptive recreation, so much of each shift would involve being out in the community.
*Must have reliable transportation and be able to pass a background check.
Scheduling- 3 opportunities available
1. Overnight position. 6-8 hour shift, generally 10-6a but can be flexible to accommodate the needs of staff. Daily rate: $130
2. AM position with a start time of 7a daily. Every other weekend is required. The start time is negotiable within reason. Wage: $19/hour
3. Everyother weekend/ PRN Wage will depend on the shift
Duties
- Assist with variable daily activities of independent living, including all personal hygeine, showering, grooming, dressing, urostomy care, transferring with a hoyer lift, administration of medication, meal preparation.
- Maintain a clean and safe environment, including light housekeeping tasks.
- Valid driver's license is required due to transportation needs (driving to appointments in client's vehicle, social outings, and other errands as needed).
Considerations:
- Experience working with individuals with disabilities is preferred -- especially younger adults with a physical disability who are actively involved in their social life, work, and community.
- Must display strong interpersonal and communication skills.
- Must possess a strong commitment to providing exceptional care, demonstrating patience and understanding in all interactions.
- Must possess strong problem-solving and critical thinking skills.
- Physical capability to perform necessary mobility assistance tasks and remain very active during care shifts.
- Valid driver's license is required due to transportation duties.
- Must be reliable, punctual, and able to work flexible hours as needed. Schedule will be determined between PCA and client.
- CNA license is a plus, but not required. Training is provided.
- Must pass a background check.
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
