The Chaplain (PRN) provides spiritual care services for patients / families during business or non-business hours on weekdays or weeknights and weekends; provides spiritual support for patients and / or families when requested; contributes to the smooth function of services for a 24 hour / 7day a week hospice care program.
Position Details
- Location : Jacksonville metro area; Duval and potentially surrounding counties
- Education : Master of Divinity; two complete CPE credits (see below)
- Hours : Flexible hours; 5-40 hours a week, dependent upon needs
Primary Responsibilities :
Business Hour Requests : Responds to requests by Spiritual Care Manager providing spiritual support for specific clients who have requested chaplain servicesNon-Business Hour Requests : (Nights & Weekends) Responds to requests from clinical triage teams or the Spiritual Care Manager providing spiritual support for patients / families in unpredictable and / or unknown environments.Provides chaplain coverage for Interdisciplinary Care Teams, including reporting on spiritual care provided, as reported by staff chaplains, and contributing to the update of Care Plans, as pertaining to spiritual care needs of clients.Provides direct services to patients and families including spiritual care in alignment with patient / families personal belief systems.Makes direct contact with the clergy and / or faith community of the patient with patient's permission, in order to facilitate requested rituals or support for specific religious / spiritual traditions and practices.Conducts religious services of prayer, worship and rituals for patients / families as appropriateAssists families at the time of a patient's death with contacting of the funeral home, disposal of narcotic medications, and with closure rituals, if requested.Makes referrals to Triage team and caseload team members for appropriate follow up.Serves as an advocate for patient / families, providing any related documentation of patient / family interventions according to Hospice policy.When working during non-business hours, communicate with Triage team and Spiritual Care Manager about requests received and interventions provided.Comply with professional, ethical standards in delivering services to patients and families.Other duties as assignedEducation & Required Licenses / Certifications :
Master of Divinity Degree with a minimum of 72 credit hours from a seminary accredited by the Council for Higher Education (www.chea.org).Completion of at least two (2) complete units of ACPE Clinical Pastoral Education (www.acpe.edu) preferably in a hospital or hospice setting.Ordination (or its equivalency) for ministry by the faith group / belief system to which the individual belongs.Specific Endorsement for chaplaincy ministry by the faith group / belief system to which the individual belongs.Experience
Minimum five (5) years experience in pastoral care, at lease one (1) year of which was in health care setting.Desired Knowledge and Skills :
Hospice philosophy and end-of-life care.Standards of practice and ethical standards for practice as a pastoral care professional.Various religious / spiritual beliefs, traditions and practices, particularly as they pertain to the delivery of health care.We are an equal opportunity employer.
