Job Summary :
The Quality and Risk Management (QRM) Program Coordinator, reporting to the Director of Quality and Risk Management, is responsible for coordinating and acquiring data from source systems specific to quality and risk management, regulatory and performance improvement metrics using methods of audits, tracers, chronologies, root cause analysis and rounding skill validation activities. The QRM Program Coordinator provides expertise and support for Quality and Risk Management functions, including abstracting, data aggregation and analysis, and medical record review for quality and risk assessment. This individual will demonstrate their expertise in quality management and performance improvement through the coordination and maintenance of quality and risk reduction initiatives to support performance improvement programs, survey readiness, and patient safety. Analyze and trends data for opportunities for improvement / process improvement. This role is expected to apply critical thinking knowledge and analytical skills to assist the Director of QRM and leadership in implementing quality improvement and risk reduction strategies and change with a strong focus on improving quality outcomes and results.
Essential Functions :
- Communicate effectively to different audiences.
- Proficient in computer skills using EXCEL, PowerPoint, MS Office, and Flowchart tools.
- Knowledgeable of High-Reliability Principles and PDCA methodology
- Use performance and process improvement (PPI), project management and change management methods to support operational and quality initiatives, improved performance and achieve organizational goals.
- Leverage the organization's analytic environment to help guide data-driven decision-making and inform quality improvement initiatives.
- Coordinate and support the organization wide processes for evaluating, monitoring, and improving compliance with internal and external requirements. Support the Director of QRM and Manager of Risk and Regulatory Management in leading the organization's processes to prepare for, participate in, and follow up on regulatory, accreditation and certification surveys and activities.
- Represent the Quality and Risk Management department in supporting and cultivating a safe healthcare environment by promoting safe practices, nurturing a just culture, and improving processes that detect, mitigate, or prevent harm.
- Coordinate activities that support compliance with organization wide voluntary, mandatory, and contractual requirements for data acquisition, analysis, reporting, and improvement.
- Engage in the healthcare quality profession with a commitment to practicing ethically, enhancing one's competence, and advancing the field.
- Support the Director of QRM and Manager of Risk and Regulatory Management in the Identification and assessment of potential risks in all aspects of hospital operations, including clinical care, medication management, environmental safety, and infection control.
- Analyze risk data and trends to identify areas for improvement, develop comprehensive risk management plans, and implement preventive measures.
- Collaborate with various departments, including nursing, medical staff, administration, and patient safety committees, to share risk information and implement risk mitigation strategies.
- Assist in the root cause analysis and incident investigations to understand the underlying causes of adverse events and implement corrective actions to prevent future occurrences.
- Support the development and delivery educational programs to staff on quality and risk management practices, safety protocols, and incident reporting procedures.
- Monitor and evaluate the effectiveness of implemented quality, risk, regulatory, performance improvement mitigation strategies.
- Maintain accurate and comprehensive records of all identified assessments and implemented initiatives.
- Stay up to date on current healthcare regulations and best practices.
- Uses AIDET in interactions with patients and family members.
- Acts with a sense of urgency when performing tasks.
- Basic unit / department maintenance such as keeping files, drawers, cabinets free from unnecessary clutter.
- Reports on any equipment and or environmental issues for repair.
- Abides by HIPAA (Health Insurance Portability and Accountability Act) regulations.
- Speaks up to stop the line and escalates potential safety events if necessary.
- Completes and attends monthly training assigned.
- May float to other areas of the hospital as needed.
