Geriatric Care Specialist

**Job Summary**

We are seeking a skilled Social Worker to join our team at Community LIFE, a Program of All-inclusive Care for the Elderly (PACE). As a Social Worker, you will play a vital role in maintaining the well-being of our participants and their families.

Our program brings together experts in geriatric medicine and care to work as a team in specialized Day Centers, providing a wide range of services designed to meet the varied needs of seniors.

Responsibilities

• Maintain a caseload of Community LIFE participants and provide psycho-social assessments and interventions related to their maintenance in the community.
• Perform utilization case management for all acute inpatient admissions and transfers.
• Work closely with healthcare professionals to develop and implement comprehensive care plans.
• Provide education and support to participants and their families on various topics related to aging and health.

Requirements

• Master's Degree in Social Work (MSW) from an accredited school of social work.
• Minimum one year experience working with the frail or elderly population, preferably in a hospital, long-term care, or community setting.
• Excellent verbal and written communication skills, with the ability to work effectively with people from diverse backgrounds.
• Able to work independently and be self-directed, with good problem-solving skills and a compassionate attitude towards older adults.
