Job Responsibilities
As a RN Case Manager, you will be responsible for assessing the needs of older adults in our community and developing care plans to meet those needs. You will also coordinate services to ensure that patients receive the care they need to live safely and independently in their own homes.

Responsibilities
- Assess patient needs and develop care plans
- Coordinate services to meet patient needs
- Attend interdisciplinary meetings and care-planning meetings
- Document patient information and progress
Requirements
- Graduate of an accredited nursing school
- At least one year of experience working with a frail elderly population
- Licensed Registered Nurse in Pennsylvania
- Valid Driver's License
