Community Living Connections, Inc. (CLC) is seeking a dedicated Community Support Manager (CSM) to oversee day-to-day operations for programs supporting individuals with developmental disabilities. Reporting to the Division Manager, this position provides direct care services to the people we support and helps to manage a program team. Key Responsibilities :
Person-Centered Support : Implement individualized care plans tailored to the unique needs and goals of each person you assist. This includes fostering personal growth, life skills, and community integration.
Daily Living Assistance : Provide support with household tasks such as cooking, cleaning, and organizing, while encouraging independence in these activities.
Program Management : Oversee the daily operations of assigned residential programs, ensuring high-quality, person-centered support.
Advocacy : Act as an advocate for individuals, supporting their goals and life aspirations.
Health & Safety Oversight : Ensure all health, safety, and individual rights regulations are strictly followed.
Collaboration : Work closely with Care Teams, medical professionals, service providers, and families to coordinate necessary services and appointments.
Administrative Leadership : Manage staff schedules, payroll, and documentation while adhering to CLC standards. Ensure compliance with state, local, and federal regulations.
Communication : Maintain open and regular communication with funders, service providers, family members, and guardians. Participate in meetings and provide updates to Division Manager and other stakeholders.
Emergency Response : Be available 24 / 7 on a rotation basis to respond to emergency situations and staff coverage needs.
What We're Looking For :
Direct Care Specialist : A desire to support individuals with developmental disabilities.
Leadership Skills : Ability to lead, motivate, and mentor staff while promoting a positive and collaborative work environment.
Person-Centered Approach : Commitment to providing individualized, high-quality services that respect the preferences and goals of the individuals supported.
Excellent Communication : Strong verbal and written communication skills, with the ability to engage with diverse stakeholders including families, staff, and service providers.
Organizational Skills : Exceptional time management and ability to manage multiple priorities while maintaining accurate records and reports.
Qualifications :
Must be at least 18 years old.
Preferred : A degree in Human Services (or equivalent) and at least 2 years of experience working with individuals with developmental disabilities.
Preferred : Supervisory or leadership experience.
Proficient in Microsoft Office and other computer applications.
Driving Requirements :
Must have access to a reliable vehicle in good working condition, a valid WI driver's license, a clean driving record, and proof of vehicle insurance that meets CLC standards.
Physical Requirements :
Ability to lift up to 50 pounds and perform various physical tasks, including standing, walking, bending, squatting, and kneeling as needed.
Specific households may require additional lifting capabilities.

Hourly Pay : $20.00 / hr base ( on call stipend and overtime pay)
