End-of-Life Nursing Specialist

Job Description

• Responsibilities:
• Conduct initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services.
• Identify and address patients' physical, psychosocial, and environmental needs through ongoing assessment and evaluation.
• Develop and implement individualized plans of care in collaboration with the patient, family, attending physician, and interdisciplinary team.
• Collaboration and Communication:
• Establish effective working relationships with patients, families, physicians, and healthcare professionals.
• Maintain accurate and comprehensive clinical records regarding patient condition, progress, and response to care.
• Requirements:
• One year of clinical experience as an RN.
• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
