RN Case Manager Opportunity
We are seeking a compassionate and skilled RN to join our team as a Hospice Case Manager.
Overview:

Key Responsibilities:
- Develop trusting relationships with patients and families as an end-of-life care expert.
- Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
- Continuously evaluate personal and professional performance, making necessary changes to increase productivity and quality of care delivered.
Hospice Care Requirements:
- Initial nursing evaluation within 48 hours of referral.
- Identify patient's physical, psychosocial, and environmental needs, reassessing as needed.
- Initiate and coordinate the plan of care, maintaining accurate clinical notes.
- Document problems, goals, interventions, and patient response to hospice care.
Qualifications:
- One year of clinical experience.
- Current RN licensure in the state of practice.
- Current CPR Certification.
- Valid driver's license and vehicle insurance.
