Community Mainstreaming is a non-profit human service agency committed to providing services to help people with intellectual and developmental disabilities lead meaningful productive lives within their community.
We are seeking Self-Direction Staff to join our team! The Self-Direction Staff will provide the oversight and support necessary to enable the program participants to independently, maturely and responsibly manage all aspects of their lives.
Location: Nassau & Suffolk County, NY
Position: Part time
Hours: Flexible
Hourly rate: Varies
Responsibilities:
- provides meaningful instructions to the individuals with skills related to living independently (such as activities of daily living, personal care, health care, community inclusion, relationship building and development of social and leisure skills, accessing community services and supports, developing independent community travel skills)
- encourages individuals to identify potential outcomes
- works with individuals to appropriately communicate with outside entities such as Care Managers, vendors, doctors, employment specialists, landlord, etc.
- provides necessary level of training and support for individuals to participate in their community
Qualifications:
- minimum 18 years old
- High School Diploma or GED
- ability to work independently
- excellent written and verbal communication skills
- valid New York State Driver’s License
- must have own transportation
- proof of car insurance and registration
- computer proficiency a must
- smartphone required
