Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized service plans, and coordinating resources to help clients achieve their goals. Duties
- Conduct thorough assessments of clients’ needs, strengths, and challenges.
- Develop, implement, and monitor individualized service plans in collaboration with clients.
- Provide crisis management support as needed.
- Facilitate access to community resources, including housing assistance .
- Mentor clients in workforce development skills to enhance employability.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Supervise and support clients during their journey towards self-sufficiency.
- Maintain accurate documentation of client interactions, progress, and outcomes.
- Collaborate with multidisciplinary teams to ensure holistic care for clients.
- Advocate for clients’ rights and needs within the community.
Qualifications
- Bachelor’s degree in Social Work or a related field; relevant experience may substitute for education.
- Experience in addiction counseling, crisis management, or social work is preferred.
- Strong mentoring skills with the ability to inspire and motivate clients.
- Familiarity with workforce development practices is a plus.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a team environment while also being self-motivated.
- Strong organizational skills with attention to detail in documentation practices.
- Commitment to maintaining confidentiality and ethical standards in all interactions.

Job Types: Full-time, Part-time
Pay: $19.90 - $23.03 per hour
Expected hours: 20 – 40 per week
