Position Summary: Community of Hope is hiring a Lead Cancer Prevention Navigator. The Lead Cancer Prevention Navigator ensures and provides adequate support, care coordination, and education when breast and colorectal cancer is detected, in order to connect patients to specialty care and treatment. This role will implement clinic processes to promote the detection of breast and colorectal cancer to include reminders to providers to do testing as well as procurement of records from other providers to ensure that all women are adequately screened for cervical center. The Lead Cancer Prevention Navigator is a full-time position based out of our Family Health and Birth Center in NE, DC. This role reports directly to our Director of Nurse Care Management & Population Health and will supervise the Cancer Prevention Navigator.

The pay rate for this position ranges between $28.36 - $29.32 per hour and the offer amount is determined by the candidate's education, qualifications, and experience . really provides their own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities:

  • Serve as a liaison with DC Health’s Patient Navigation Program along with DC Primary Care Association to support the standardization and coordination of cancer prevention interventions across the cancer prevention navigator team at COH.
  • Manages a panel of patients whose breast or colorectal cancer screening results are positive and provides care coordination and navigation support to ensure follow-up and connection to additional diagnostic testing and treatment as needed.
  • Facilitates and schedules patient appointments/referrals with appropriate provider(s) for breast and colorectal cancer screening, diagnostics, and treatment. Attends appointments with patients on an as needed basis.
  • Identifies patient barriers to care such as transportation, housing instability, insurance, and language barriers, and refers patients as needed to additional resources at COH and other partner organizations.
  • Ensures that COH patients complete breast and colorectal cancer screens or have recently had screens done and the records have been received by COH. Reviews patient records and flags providers to ensure that screening is complete.
  • Makes confirmation calls/emails to patients regarding future appointment dates and assists patients in arriving to scheduled appointments on time and prepared.
  • Responds to patient inquiries and assists patients with the completion of paperwork.
  • Educates patients on care processes and provides counsel to reduce anxiety and fear.
  • Educates patients on the importance of breast and colorectal cancer prevention and their screening options for early detection
  • Supports clinic quality improvement (QI) efforts regarding cervical, breast and colorectal cancer screening, prevention, and referrals. This includes participating and leading Plan-Do-Study-Act (PDSA) change cycles.
  • Using Electronic Health Record (EHR) system, Relevant and CRISPtracks navigation services, records messages from patients, and contributes to clinic tracking workflows; including obtaining results from external facilities.
  • Tracks appropriate patient-level quality outcomes for program monitoring and evaluation.
  • Attends relevant stakeholder and advisory meetings to support overall program implementation and evaluation.
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.
  • Collaborates with the Director of Nurse Care Management and Population Health, who serves as the program manager, to identify and support improvement activities for the program.
  • Documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record within 2-3 business days.
  • Uses all mandatory, structured fields and electronic Direct/P2P referrals as required in eCW.
  • Meets with supervisee regularly and provides real-time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns promptly as needed.
  • Applies principles learned through diversity, equity, and inclusion (DEI) trainings and/or trauma informed care trainings (TIC) and leads with a DEI and TIC lens when appropriate.
  • Reviews the performance of team member in a timely manner, including completing 90 day and annual performance evaluations per their due dates, documenting any issues or changes, providing recommendations for professional development and/or training opportunities, and recognizing staff when appropriate
  • Partners with the Talent Management team regarding personnel matters including but not limited to, terminations, leaves of absence, training, onboarding, and other COH handbook policies.
  • If applicable, actively recruits for vacancies with a sense of urgency. This includes reviewing resumes, phone screening, interviewing, and completing references. This also includes partnering with Talent Management for assistance in an effort to fill vacancies promptly.
  • Takes initiative and makes decisions within the scope of work. Assesses situations independently, thinks critically, and takes action to address issues or make useful recommendations while maintaining a solution-oriented lens.???
  • Completes other health information management duties as assigned or as needed per HIM department needs. *
Requirements:

Minimum Qualifications:

  • Associate or Bachelor’s degree or equivalent experience required.
  • BLS Certification required.
  • 1-2 years of leadership experience preferred with health care clinic operations or at a community-based organization; preferably a Federally Qualified Health Center
  • Ability to work collaboratively with clinicians, internal staff, and external organizations
  • Commitment to addressing health disparities and barriers posed by social determinants of health
  • Excellent time management and coordination skills, with the ability to set priorities given multiple work assignments
  • High degree of independence, flexibility, initiative, and commitment
  • Strong communication skills with diverse populations
  • Proficiency with Microsoft Word and Excel required
  • Experience with an Electronic Health Record; eClinical Works preferred
  • Will be required to travel to other sites as needed.
