- Support and encourage individuals to enhance their independence.
- Regularly meet with waiver participants in their residences, the community, or an office setting.
- Collaborate with the Case Manager to create and implement programs tailored to the individual's requirements.
- Provide ongoing support for community integration efforts.
- Document individual progress towards acquiring community integration skills.
- Establish a support network between the individual and the community.
- Ensure medical consultation forms are completed by physicians after each medical appointment.
