Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are now hiring a Full-Time Qualified Intellectual Disabilities Professional (QIDP) in Manhattan, NY . The Qualified Intellectual Disability Professional assists the Executive Director in developing and maintaining individual service plans that meet the needs of people with disabilities and state and area program guidelines.
Starting Rate : $24 / hourly
Responsibilities
- Develop individual service plans (ISP) / Person Centered Plans (PCP)
- Coordinate admission and discharge activities of participants
- Prepare monthly goal progress notes and billing justification forms
- Maintain and analyze all incident / accident reports
- Coordinate behavioral intervention plans, to include routine and emergency services
- Monitor medication procedures, administration, and documentation
- Monitor staff compliance with plan implementation and licensing compliance
- Evaluate staff performance
- Additional tasks and responsibilities may be assigned
- Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
