We are seeking a compassionate Foster Care Case Worker to join our team. In this rewarding role, you'll have the opportunity to positively impact the lives of children in the foster care system by placing and supervising them in independent living settings, working closely with families, and developing personalized service plans.
Key Responsibilities:
- Case Planning & Case Management: Develop, implement, and review individualized service plans for children and their families. Ensure that the needs of foster children are met and that plans are regularly updated.
- Independent Living Placement: Select appropriate placements for children in foster care and ensure the appropriate supervision of placements in independent living settings.
- Life Skills Training: Provide guidance and training in essential life skills to foster children to promote independence and personal growth.
- Collaboration & Coordination: Work closely with foster children, their families, and treatment teams to ensure coordinated care. Regularly communicate with stakeholders to ensure successful outcomes.
- 24/7 Availability: Be available for on-call support and emergencies, ensuring the well-being of children at all times.
- Bachelor's degree in Human Behavioral Science, Social Work, Psychology, or a related field (25% of coursework must be in human behavioral sciences).
- Valid and current Driver's License with a reliable mode of transportation.
- Strong case management and organizational skills with a focus on child welfare and family support.
- Excellent communication and interpersonal skills, with the ability to work effectively with families, children, and treatment teams.
- Ability to work flexible hours, including evenings and weekends as required.
- Proficiency in Microsoft Office and other case management software.
- Experience in foster care, youth development, or adolescent services.
- Familiarity with independent living programs and life skills training.
- Experience working in a community care or social services setting.
- Knowledge of child welfare regulations and best practices in foster care case management.
