Participant Navigator, Part-Time (16-20 hours weekly)
The Participant Navigator at Community PACE in Newaygo, MI, plays a vital role in assisting patients with Medicaid Redeterminations, maintaining PACE eligibility, and building strong relationships with local MDDHS case workers. This position involves collaborative work with patients, their families, and representatives to ensure continued access to Medicaid and PACE services. The ideal candidate will have strong communication skills, a passion for helping others, and a commitment to improving access to healthcare for individuals in our community.
Key Responsibilities:
- Medicaid Redeterminations: Assist patients through the Medicaid redetermination process, ensuring all required documentation is submitted accurately and in a timely manner.
- Relationship Building: Develop and maintain positive working relationships with local MDDHS case workers to support the needs of PACE participants.
- Eligibility Coordination: Work closely with patients and their representatives to help establish and maintain Medicaid and PACE eligibility, offering guidance and support throughout the process.
- Support and Advocacy: Act as an advocate for participants, addressing any concerns or questions they may have regarding their Medicaid or PACE benefits and services.
- Documentation and Reporting: Ensure all interactions and updates are accurately documented in participant records, maintaining confidentiality and compliance with all relevant policies.
- Collaboration: Partner with other Community PACE team members to provide seamless support and ensure the overall success of the program and participants’ care.
Qualifications:
- High school diploma or equivalent required; additional healthcare or social services education preferred.
- Previous experience working with Medicaid or PACE programs is a plus.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
- Ability to build relationships with community partners and navigate complex systems.
- Detail-oriented and organized, with the ability to manage multiple tasks and deadlines.
- Familiarity with HIPAA regulations and a commitment to maintaining confidentiality.
- Compassionate, patient-focused attitude with a strong desire to help others.
This is a part-time position with flexible hours, ranging from 16-20 hours per week. The Participant Navigator will be based out of our home office in Newaygo, MI, and may be required to visit patients or local agencies throughout our service area as needed. The Community PACE service area includes Lake, Mason, Mecosta, Newaygo, Oceana, Osceola and Montcalm counties.
