JOB TITLE: Children’s Disability Specialist
FLSA STATUS: NON - EXEMPT
SALARY: $42,500
POSITION SUMMARY:
This is a clinical based position responsible for identifying youth that potentially meet the criteria for Agency for Persons with Disabilities (APD), navigating the process with those selected youth to get them onto the waitlist, assisting with needed services and placement opportunities.

POSITION SUMMARY:
This is a clinical based position responsible for identifying youth that potentially meet the criteria for Agency for Persons with Disabilities (APD), navigating the process with those selected youth to get them onto the waitlist, assisting with needed services and placement opportunities.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Collecting documents/records, reviewing, and assessing for a qualifying diagnosis, identifying further evaluations (if needed), communicating with case management to accomplish needed evals and submitting packets to APD
- Review/monitor shelters, staffing notes, evaluations and/or emails where language is stated that a youth may have a diagnosis that meets criteria for APD. Proceeding with the initiative and action steps to begin the APD process
- Review all APD clients/pending clients in FSFN and APD tracker, review monthly work/progress and update APD spreadsheet
- Respond to changes with a youth as it relates to their position on the APD waitlist which involves completion of Budget waiver forms and updating APD of placement changes, i.e. providing address changes,
- Submit APD crisis requests as needed which involves immediate placement and service request needs for a youth
- Communicate with APD providers regarding acceptance of our children into their group homes, assist in coordination of the move along with case management. (Involves coordination of conference calls, collection of records, communication with contracts department regarding payment, etc.)
- Coordinate and facilitate monthly meetings with APD/DCF to review clients and pending clients
- Coordinate and facilitate an internal or stakeholder MDT staffing for youth on the APD waitlist that need attention surrounding their APD status, services and/or placement needs, behavioral needs and youth aging out of foster care
- Participate in DCF and MDT staffings regarding APD clients or potential APD clients as needed and complete follow up
- Communicate with APD providers regarding payment/services issues
- Assist in arranging testing and coordinating ABA services for children for APD purposes as needed
- Ability to cross-train with educational liaison position
- Knowledgeable of child welfare and case management systems preferred
- Knowledge of Autism testing process and sites; referral procedure and testing steps preferred
- Perform special projects as assigned

EXPERIENCE/PERFORMANCE REQUIREMENTS (Knowledge, skills and abilities):
- Minimum of three years’ experience working with at-risk children and families
- Knowledge, training, interest in working with adolescents, family resources and educational systems.
- Very good organizational, written and verbal communication skills.
- Ability to accommodate a flexible work schedule, including evenings and weekends.
- Ability to assess and problem solve quickly and make decisions.
- Ability to handle confidential information appropriately.
- Ability to assess appropriate intervention strategies.
- Knowledge of community resources.
EDUCATION REQUIREMENTS
Bachelor’s Degree from an accredited university in Human Services related field
LICENSES AND CERTIFICATIONS
Possess a current Background Clearance Screening Letter as required by DCF.
If local travel is required, a Valid Florida driver’s license and documentation of current automobile insurance, with $100,000/$300,000 bodily injury liability limit, is required.
PHYSICAL DEMANDS AND WORK ENVIRONMENT
Employee will work in an office/clerical environment. The employee will work predominantly seated with recurrent need to walk, stand, and bend from the waist. Occasional light lifting, stooping, and climbing may be required. Occasional local travel.
