What are the responsibilities and job description for the Community Health Worker position at Community Partnership of Southeast Missouri?
POSITION SUMMARY:
The Community Health Worker (CHW) is responsible for helping clients and their families navigate and access community services, adopt healthy behaviors and other resources.
The CHW provide support to local healthcare organizations and systems through a cohesive community outreach approach. CHW activities will promote, maintain,
and improve the health of clients and their family. CHW will provide social support, health education, community health needs, advocates for individuals and provide services such as
simple wound care, blood pressure screenings and basic first aid.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• CHW is responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.
• CHW will provide ongoing follow-up and goal setting with clients/families.
• CHW will follow – up with clients via phone calls, home visits and visits to other settings where clients can be found monthly. A minimum of two forms of contact should
take place with clients once a month. Face to face visits each month is mandatory as one of the two forms of contact.
• CHW will help patients attend medical appointments and set personal health related goals.
• CHW will provide clients with community referrals for services when appropriate.
• CHW will help clients connect with transportation resources and provide appointment reminders in special circumstances. Transporting clients is based on CPSEMO’s
transportation policy.
• CHW is not allowed to work with their own family members.
• CHW will display professional working relations with clients, visitors and staff,
• CHW will effectively communicate CPSEMO’s mission.
• CHW will work closely with medical providers to help ensure clients have
comprehensive and coordinated care plans.
• CHW will be familiar with community resources appropriate to needs of clients/families.
• CHW is responsible for providing consistent communication to medical providers.
• CHW will act as an advocate and liaison between the clients/family and community service agencies.
• CHW will record client’s care management information in Apricot no later than 24 hours after client contact.
• CHW will attend regular staff meetings, trainings and other meetings, as requested.
• CHW will be responsible to go out to recruit clients weekly. CHW will recruit at health care facilities, fitness and wellness expos, gyms etc.
• CHW will manage assigned caseload of clients.
• CHW will maintain HIPPA compliance at all times.
NON-ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Performs other duties as assigned.
