What are the responsibilities and job description for the Lead Case Manager position at Community Reconstruction Inc.?
Summary: case management team ensures effective delivery of the Rites of Passage Program model and a seamless delivery of services in a manner that supports the individualized needs of the youth and his family. Case Managers are responsible for the case management of youth participating in the ROP Program through administering assessments, developing service plans, maintaining progress notes, linking youth and family members for services, and collaborating with Youth Coaches in addressing the needs of youth.
Responsibilities:
- Administer a Needs Assessment, PYDI, YBLCS (within 7 days when applicable) for each youth admitted to the ROP Program within fourteen (14) days of admission.
- Develop a Prevention Services Plan with twenty-one days of admission that includes feedback from the youth, family members, school personnel, juvenile probation officer, Life Coach and other professionals involved in the case.
- Maintain an accurate and current case file via Progress Notes, and updates/modifications to the Prevention Services Plan.
- Administer a Pre- and Post-Test at admission, six (6) months, and within seven (7) days of discharge.
- Refer youth to local service professionals with the ability to meet youth needs not available through the ROP Program and monitors youth participation in those services.
- Schedules and facilitates monthly Prevention Services Planning meetings.
- Partner with service professionals to identify and meet the needs of youth participating in the ROP Program.
- Supervise case management and provide ongoing training in effective case management via weekly review of youth files.
- Coordinate the scheduling of individual progress meetings with the youth and service professionals including school personnel (teachers and guidance counselors), the Life Coach, juvenile probation officer (as applicable), and family members.
- Link youth with services not available through the ROP Program (e.g. behavioral health counseling) and monitors the youth’s active participation in these services.
- Review completed Needs Assessment, Prevention Services Plan, and participates in meetings with the youth and his parents.
- Meet with the ROP Senior Leadership team to identify opportunities for enhancement and barriers to successful service delivery.
- Manage case load of youth, when necessary.
- Resolve conflicts and grievances between youth and ROP staff.
- Review referrals to ensure completeness/appropriateness for admission to the Program.

Education and Experience
- Bachelor’s degree from an accredited four-year college or university in psychology, social work, law enforcement, or a related human services field.
- Two (2) years of professional experience supervising non-clinical staff. Three (3) years of experience managing publically- funded contracts.
- Three (3) years of professional experience working with at-risk, dependent or delinquent youth, preferably in a case management capacity.
- Strong organizational, leadership and communication skills.
