Job Summary:
The Case Manager provides aftercare services to clients by enrolling them in educational/vocational programs, post-discharge planning, and job placement needs.
Essential Duties and Responsibilities:
- Provides case management- aftercare services minimally maintaining weekly contact with each client on his/her caseload.
- Assists program participants in enrolling in needed educational/vocational program, as appropriate.
- Provide services to clients including visits to home, work, and other fieldwork.
- Assists program participants in post-discharge planning to minimally address housing, clothing, personal care, medical, psychiatric, and job placement needs.
- Provides complete and timely documentation of all services within clinical records and ensure records meet DCFS regulations, accreditation standards, CRC policies, and contractual requirements.
- Coordinates post-treatment services with other service providers, including family services as needed.
- Develops & monitors a Client Continuing Plan with each assigned program participant to ensure clients to ensure the plan is in compliance standards.
- Minimally leads one- Aftercare Support Group meeting each week.
- Is knowledgeable of and regularly provides training on, addiction relapse triggers and prevention techniques in aftercare group.
- Provide transportation for clients as needed.
- Establishes and maintains working relationships with community resources.
- Maintains a strong & functional linkage to the Corrections Therapeutic Community to ensure a smooth transition into the community after release from jail.
- Complies with all confidentiality regulations and policies.
- Participates in weekly staff meetings.
- Must work effectively in an atmosphere of dignity and respect for clients and co-workers.
- Participate in internal training programs.
- Participate in Quality Improvement Program.

Education and Experience Requirements:
- A Bachelor’s Degree in a behavioral health or human service with at least 2 years of relevant experience.
- Acceptable professional certification as an addictions and/or criminal justice professional preferred.
- A valid Florida driver’s license and a good driving record are required.
- Experience in a social service environment is preferred.
Knowledge/Skills/Abilities:
- Must be able to provide own transportation.
- Must be eligible for Level 2 DCF Background Screening approval.
- Must be able to communicate effectively verbally and in writing. Computer competency required.
- Must possess excellent organizational skills.
- Must have the ability to handle confidential information appropriately.
- Ability to interact with people from diverse cultural, economic and ethnic backgrounds.
- Ability to set appropriate boundaries and limits with clients.
- The ability to correctly document and complete services in the Electronic Medical Records system.
Job Type: Full-time
Pay: $37,000.00 - $39,500.00 per year
