Purpose: At VONA, we believe in the power of purpose and passion in everything we do. Our Field Case Manager will facilitate and coordinate the rehabilitation needs of the injured worker in a timely and cost-effective manner; promoting optimal outcomes, advocating for the recovery from injury or illness, physical and emotional well-being, return to work and normal activities of daily living.

Core Responsibilities:
- Assessing the injured workers’ medical issue and coordinating the treatment plan.
- Maintaining phone and/or digital contact with all parties involved in a timely manner.
- Coordinating and attending injured workers’ appointments as needed.
- Work with employers on job duty modifications, based on medical limitations and the employee’s functional assessment.
- Explain instructions given by physicians and therapists, along with answering any questions the injured worker may have to facilitate his/her return to work.
- Serve as an intermediary to interpret and educate the injured worker on his/her injury and the treatment plan established by the case manager, physicians, and therapists.
- Acquiring and maintaining knowledge of developments in the medical case management field.
- Keep personal licenses and accreditation current and in good standing.
- Deliver monthly progress reports on a timely basis.
- Document your time for case management duties as requested by VONA.
Qualifications:
- RN License required.
- CCM Certification (Certified Case Manager) preferred.
- Prior Case Management experience required.
- Multilingual proficiency preferred.
- Strong understanding of medical terminology.
- Demonstrated experience in hospital settings.
- Familiarity with HIPAA regulations.
