What are the responsibilities and job description for the Case Manager position at Community Rehabilitation Center, Inc.?
Objective:
The primary function of case managers is to advocate for clients/support systems. Case managers understand the importance of achieving quality outcomes for their clients and commit to the appropriate use of resources and empowerment of clients in a manner that is supportive and objective.
Description of Duties and Responsibilities:
- Serves as a self-driven program staff that is responsible for full care coordination.
- Provides intensive and purposeful case management, such as applying for SNAP, SSDI, SSI, and other benefits available for each individualized participant.
- Works directly with CRC and its sponsoring partners to coordinate services.
- Conducts thoroughly organized presentations and other outreach activities in the poverty stricken communities.
- Participates, coordinates, and/or attends community events such as community forums and health fairs, as a collaborative representative of CRC.
- Works in close proximity with counselors to ensure coordination of care is maximized.
- Monitors successful linkage in the community using project services.
- Participates in SOAR mandatory trainings and updated meetings related to the program to build and maintain current knowledge based on the awareness and prevention in the specific area of homelessness, substance misuse, and co-occurring disorders.
- Provides targeted services to homeless or at-risk of homelessness individuals who have a SUD and/or COD.
- Works closely with the program evaluator for the confidential transmittal of data and programmatic information according to HIPPA.
- Collects data utilizing SAMHSA approved GPRA as the data tool.
- Creates open dialogue among participants and care coordination services per need of the program participant.
- Connects participants to HUD and housing assistance programs and/or shelters.
- Documents all communication with program clients using an in-house approved tool for case notes (DAP, SOAP, etc.).
- Serves as the point-of-contact for program participants who does not have a designated emergency contact.
- Serves as the liaison for program participants who do not speak English.
- Provides accurate, updated, and precise information that has changed for the program participants to the local agencies that care has been coordinated.
- Creates an open door policy for all programmatic participants.
- Organizes scheduled meetings with participants and care coordination providers.
- Participates in conducting follow-up and discharge interviews for each of the program participants.
Qualifications for Position:
- Bachelor’s level degree in the field of behavioral health or strongly related field preferred.
- HIV 500/501 training, screening, and counseling with testing preferred.
- One year of experience required in providing services to the homeless and/or those at-risk of homelessness.
- Two years of care coordination training and diverse and intense case management.
- Harm reduction training principles and motivational interviewing preferred.
- Any specific combination of experience and education equivalent to the above requirements.
Supervisory Relationship:
Report to Project Coordinator
Skills and Knowledge Required:
- Pre-requisite knowledge of barriers and/or challenges that have been faced when assisting the homeless population.
- Previous experience connecting homeless clients with substance misuse/mental health disorders to support and medical care services.
- Previous experience demonstrating leadership characteristics while working comfortably with a diverse population, applying sensitivity to situations concerning physical or mental disabilities of any kind.
- The mindful and thoughtful ability to make logical and sound decisions in accordance to the agency, local, and state guidelines process and procedures outlined in the grant.
- Exceptional and effective communication skills, both verbally and written.
- Multicultural (Bilingual) English/Spanish preferred not required.
- Maintaining strict confidentiality in respect to HIPPA, while adhering to the professional standards as defined by the agency’s policies and procedures.
- Applicable ability to take initiative and work as a team at all times.
