The Program Coordinator of Day Habilitation reports directly to the Director of Day Services and is responsible for providing oversite to all Day Habilitation participants and employees within their assigned caseloads.
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO
- Provides hands on daily oversight of program operations as it pertains to participant program goals, meaningful activities, employee interactions and community engagement,
- Provide leadership for staff to facilitate the implementation of person-centered individualized plans and services, and ensure regulatory compliance is maintained at all times.
- Ensure assigned caseload maintains compliance with operating regulations and governing procedures for billing integrity within the agency EHR system, and that all daily records and supporting documents are up to date ie: LOC, Life Plan, Medical, Psychological, Psychosocial etc
- Ensure a clean, safe, regulatory compliant physical plant environment, and assure completion of electronic records in compliance with Medicaid standards.
- Prepare staff for OPWDD recertification audits, internal audits, and assist QA with all follow up to Corrective Action Plans.
- Provide ongoing coaching to all Direct Support Staff, assist with personnel concerns, and ensure there is adequate staffing at all times.
- Participate in employee recruitment, retention, recognition, and staff training programs.
- Maintain open communication with family, parents, advocates, correspondents, and any outside agencies related to services for the program participants.
- Participate in CRSI program related committees as assigned (IRC, HRC, A&D, QA/Compliance etc.).
- Assist in the coordination and completion of Life Plan meetings, Behavioral Intervention strategy meetings and all other relevant discussions for program participants within assigned caseloads.
- Complete, review and submit electronic payroll for assigned staff.
- Other duties as assigned by the Agency and/or Executive Director
Pay Rate: 50,000 - 54,0000annually.
Qualifications
- BA/BS in the Human Services or related field, MA preferred.
- Three to Five years’ experience in an OPWDD Residential setting working with people with Intellectual and developmental disabilities.
- Three years’ supervisory experience preferred.
- Excellent verbal, written, and technical communication skills.
- Valid NY State Driver’s License is required.
- Prior experience using electronic billing systems is preferred.
Salary : $50,000 - $54,000
