POSITION RESPONSIBILITIES
- Completes file review for eligibility and enrollment and communicates potential program participants with Program Coordinator
- Participates in, writes, and implements Staff Action Habilitation Plans with oversight by the Program Coordinator.
- Ensures the staff implementation of plans and documentation of daily notes and monthlies are completed correctly. Reviews for accuracy.
- Ensures staff understands and promotes the Person-Centered Approach.
- Responsible for staff scheduling, arranging coverage and providing direct care services in the absence of staff.
- Completes the hiring, training, evaluating, and supervision of program staff including evaluating staff performance and provides feedback to staff and, as necessary to the Program Coordinator. Ensures completion of required in-services and training for staff.
- Works with Clinical Staff when necessary (nurse, behavior specialist, and psychiatrist) to ensure implementation and documentation of medical and behavior plans.
- Provides assistance with acquisition, retention or improvement of self-help and socialization skills.
- Develops, coordinates, and assists with programmatic activities. Locates community-based events to promote integration. Encourages program participants to be good neighbors, responsible citizens, join local clubs and support their community.
- Purchases supplies for the program as needed.
- Manages petty cash and gas reimbursements where applicable.
- Arranges and monitors transportation for participants with the internal transportation department or external transportation programs when necessary. Uses knowledge of NFTA system and encourages mobility training where appropriate.
- Ensure service documentation is submitted in time for billing.
- Completes monthly attestation and forwards it to Program Coordinator.
- Ensures participants satisfaction and input occurs with program activities, curriculum and goals.
- Receives and reviews minor, reportable, and serious reportable incident reports, and follows up with Program Coordinator for direction and processing.
- Maintains contact with family and/or IRA staff. Notifies appropriate parties of concerns as they arise.
- Ensures compliance with all OPWDD certification quality assurance and fiscal requirements.
- Conducts fire safety training as necessary.
- Monitors and manages units of service to achieve objectives for HCBS funding source. Coordinates program and funding modifications within Day Habilitation Programs.
- Provides follow-up services and referral to discharged individuals as needed.
- Completes data entry into Therap and assists with maintaining the database to ensure individual files are current.
SUPERVISORY RESPONSIBILITIES
Provides supervision of Assistant Managers and Certified Day Habilitation DSPs.
