The Residential Site Manager works closely with their Program Coordinator to provides day-to-day operations of their assigned site; this includes supervision of the Assistant Site Manager and assigned DSPs.
POSITION RESPONSIBILITIES
- Acts as leader and mentor within the department. Serves as a role model in Living the Mission. Displays ethical decision making.
- Models and promotes Person Centered Planning, considering the dreams and goals of individuals being served.
- Models, prompts, discusses and proactively plans with residential DSPs for active treatment of people served.
- Hires, orientates, trains, and supervises site staff in all areas of job performance including ongoing assessment of service delivery as well as knowledge of environmental modifications pertaining to the IRA for individual safety with oversight by the Program Coordinator.
- Coordinates scheduling and assignment of site staff to ensure safety and minimum staffing pattern are always met.
- Works shifts as DSP as needed.
- Based on the needs of those receiving services, DSPs will also be responsible for housekeeping, cooking, laundry services, general maintenance of the worksite; and providing physical support with daily activities, personal hygiene, including bathing, dressing, getting in/out of bed, using the toilet, walking, standing, or exercising.
- Provides transportation to individuals when necessary.
- Works in a cluster system (approximately four houses) for on-call rotational support; provides unscheduled on-call support when assigned due to staffing needs or emergent situations.
- Completes all tasks as outlined on the Site Manager Reference Guide or checklist.
- Coordinates Action Planning Meetings as required.
- Implements SCIP-R when necessary.
- Ensures assignment, completion and documentation of required in-service training for Residential Staff.
- Manages expenditures of petty cash, food stamps and individual personal funds.
- Ensures provision of or acquisition of clothing, food and household items.
- Provides supervision and assistance to individuals to ensure their health and safety including personal care, dispensing of medications, financial training and dietary needs.
- Completes data entry into Therap and assists with maintaining the database to ensure individual files are updated and current.
- Assists with creation of Residential Habilitation Plan as part of the Support Team and helps individuals achieve valued outcomes. Ensures plans and services are personalized based on individuals’ needs and dreams.
- Ensures the implementation and documentation of Residential Habilitation plans (include monthly notes, daily notes and outcome records) as well as weekly case summaries, case files and med logs.
- Ensures the ongoing safety of all staff and individuals by maintaining a safe environment as well as reporting any concerns whether through staff incident reports, safety plans, emails etc.
- Ensures protection and adequate care of individuals through review of Behavioral Incident Reports and Agency Incident Reports.
- Responsible for notifying the Program Coordinator of Reportable and Serious Reportable Incidents and Allegations of Abuse, completing follow up as directed.
- Dispenses medications to individuals as prescribed. Coordinates with the pharmacy, Service Coordinators, nurses and doctors to ensure adequate supplies of medications are readily available.
- Works with Clinical Staff to ensure implementation and documentation of medical and behavior plans. Assists Behavior Clinician with ensuring behavioral plans/protocols are complete and meet the needs of individuals.
- Provides staff training regarding behavioral plans and interventions, ensures completion of behavioral reports and ensures follow through with the Behavior Clinician.
- Ensures compliance with all OPWDD certification requirements.
- Ensures routine maintenance and repairs of the Site and Site vehicle.
- Coordinates and schedules fire drills at site.
