Job Description:

You will work closely with pharmacy, physicians, and other healthcare providers to ensure seamless care coordination. Your duties will also involve assisting individuals with prescribed treatments, educating them on medication, physical health, and self-care, and providing first aid and acute care as needed. Additionally, you will schedule and attend medical appointments, provide direct care as required, and educate coworkers on physical health matters for consistency in care.
Required Skills and Qualifications:
To be successful in this role, you will need a valid LPN license in the state of Pennsylvania, a willingness to accompany individuals into the community, and flexibility to work non-traditional hours. You must also have a valid driver's license, good driving record, and access to a reliable vehicle for transporting individuals to appointments and trainings.
