What are the responsibilities and job description for the Qualified Mental Health Professional/Case Manager position at Community Support Agency?
Community Support Agency is seeking a Qualified Professional to join our professional staff and community of stakeholders dedicated to improving the quality of life for all those we serve.
ESSENTIAL DUTIES:
- Provides support/case management services for those consumer whose treatment/rehabilitation needs require significant linkage and coordination of components in the service system. This involves ongoing contacts with medical and psychiatric consultation, the schools, hospitals, group homes, the Department of Social Services, and others as required, to insure that needed services are in place and to monitor the effectiveness and continued need for services.
- Provide “first responder” crisis response, as indicated in the Person Centered Plan, 24 hours a day, 7 days a week, 365 days a year to recipients of this service.
- Develops, implements, and revises the Person Centered Plan.
- Actively involved in providing services, consults with agencies, physicians and primary therapist.
- Provides support/case management services in the areas of academic and educational placement, community support services, housing, financial needs, home management, child care needs, medical care and treatment compliance.
- Convenes the Child and Family Team meeting monthly and includes all natural and paid supports in this.
- Provides educational and supportive counseling related to the importance of treatment compliance.
- Assists customer with skill building activities to include: daily and community living skills, socialization skills, adaptation skills, development of leisure time interests and activities, symptom management skills, wellness education, substance abuse education, and work readiness as called for in the Person Centered Plan, as necessary.
- Acts as a community liaison by forming a network with agencies, families, and churches that would benefit the consumer.
- Acts as an advocate for consumer in legal matters as the need arises, per supervisor’s direction.
- Safeguards the confidentiality of the consumers and his/her medical condition.
- Prepares and maintain all necessary documentation, reports, and records in a timely manner to facilitate internal and external managed care and quality assurance requirements, e.g. LME.
- Participates in Child Specific Team, Family Assessment and Planning Team , and other interdisciplinary, diagnostic, or planning meetings
- Participates in the MH/DD/SAS Annual Evaluation, surveys, Quality Assurance activities, Medicaid and/or contract audit.
- Defuses current crisis, evaluate its nature, and intervene to reduce the likelihood of a recurrence;
- Any other duties deemed necessary to meet MH/DD/SAS Program needs.
