Health Care Coordinator - RN

Community Systems, Inc. (CSI) is seeking a Full-Time Health Care Coordinator to join our care team. The Health Care Coordinator will provide medical oversight to adults with disabilities in their own homes in the state of Delaware. Community Systems, Inc. (CSI) is a non-profit organization offering residential and related support to persons with developmental disabilities.

• Active licensure in the state of Delaware as a Registered Nurse
• At least two-years recent nursing experience in an acute care or long-term care setting
• Strong nursing assessment and decision-making skills
• Experienced working with individuals who have intellectual disabilities preferred.
• Ability to work independently and manage a flexible work schedule.
• Excellent written and oral communication skills, computer/word processing skills
• CPR certification and physical management techniques certification
• Child Abuse Clearance, Adult Abuse Clearance, Criminal History Checks, Drug Screen, and proper documentation to work in the United States.
• Applicants must possess a minimum of two years driving experience and hold a valid U.S. vehicle driver’s license.

• Essential Functions –

  • Provide overall healthcare coordination to persons with disabilities in their own homes. This includes establishing health profiles, identifying health risks and ensuring that persons supported have the opportunity to choose healthy lifestyles.
  • Provide leadership, guidance and training for staff to effectively maintain the health status of persons supported and to ensure accurate and timely implementation of medical orders.
  • Oversee the creation and maintenance of persons’ medical records to ensure compliance with state and internal regulations and policy.
  • Participate as a member of an interdisciplinary team to advocate for the persons supported.
