GENERAL RESPONSIBILITIES:
- Provide Target Case Management and Wraparound Facilitation to persons assigned to caseload.
- Facilitate meetings with treatment/wraparound teams.
- Develop comprehensive treatment plans for persons in service.
- Coordinate, monitor and/or complete all waiver paperwork, releases, and assessments.
- Work hours are flexible to meet the needs of the person and families served.
- Monitoring of progress of treatment plans and interventions and making changes as necessary.
- Coordination of care with community partners.
Qualifications
MINIMUM QUALIFICATIONS:
- Age 21 years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.
- Excellent interpersonal and teamwork skills
- Excellent organizational and time management skills
- Interest and investment in working with children who experience Severe and Emotional Disturbance (SED) and their families.
- Have completed or will complete any required training within six months of employment.
- Must have a current and valid Kansas driver’s license.
