Geriatric Care Manager / Social Worker

Salary: $80,000 to $90,000 per year/ Based on education and experience.
Position Overview: Are you passionate about enhancing the lives of older adults? We want to meet you! Compass Care, LLC, a private concierge home care provider, is seeking a Geriatric Care Manager/Social Worker to join our team. This role is ideal for professionals are passionate about enhancing the lives of older adults and who are committed to hospitality, the highest standards of care, creative problem-solving, and helping our clients remain safe, joyful and independent in their homes. This field-based client-facing role requires local travel within our service areas. When not in the field, work is done from our Stamford, CT office or remotely. The ideal candidate will be highly motivated, detail-oriented, possess strong communication and interpersonal skills, and be a creative, forward-thinking team player. Qualifications and Education Requirements:
· Master’s or bachelor’s degree in social work (LMSW), occupational therapy, or related healthcare field.
· NACCM CMC certification preferred.
· Minimum of 3 years of work experience working with older adults.
· Proven experience in business development and marketing in healthcare or a related industry (preferred).
Job Duties:
· Home Assessments
· Assist Director with Onboarding of New Clients
· Ongoing Coordination of Care
· Relationship Building with Referral Sources
Key Skills:
· Communication: Strong verbal and written communication skills to interact with clients, families, and healthcare professionals.
· Empathy and Compassion: A deep understanding of the emotional and psychological aspects of aging, with a compassionate approach to care. Ability to connect with elderly clients and provide comfort during challenging times.
· Problem-Solving: Ability to think quickly and address issues that arise in a client’s care plan. Strong critical thinking skills to make informed decisions for clients in various situations.
· Time Management: Ability to prioritize tasks and manage time effectively to meet clients’ needs.

· Keep track of numerous tasks and responsibilities simultaneously.
· Ability to assist with sales and marketing with referral sources and other healthcare professionals.
· Prioritize tasks and manage multiple responsibilities in a dynamic environment.
· Exceptional communication and interpersonal skills, both written and verbal.
· Ability to work independently as well as collaboratively within a team environment.
· Available to work weekdays and outside regular hours when needed.
· Willing to work beyond the regular schedule if needed.
· Minimum of 2 references from current or past employers.
· Must use your own car.
Technical Skills:
· Proficient in MS 365 Suite (Outlook, Word, Excel, PowerPoint)
· Strong computer and smartphone skills, with the ability to learn new technologies quickly.
