JOB SUMMARY:
The Social Worker collaborates with colleagues and other members of the inter-disciplinary team to determine a safe and appropriate post-acute plan for patients. Completes psychosocial assessments and works with patients and families to facilitate these discharge plans. Provides crisis intervention/counseling for patients and families around a multitude of complex issues, including but not limited to issues of health, illness, dying, new diagnosis, homelessness/housing, fetal demise, developmental delays/birth defects in children. Provides drug and alcohol counseling and connects patients and families to appropriate community referrals and resources. Provides assistance and planning around Advance Directives, as well as assistance surrounding issues with insurance. Provides on-going assessments of personal needs, provides counseling, referrals and connection for assistance with; rent, utilities, medications, medical supplies, clothing, food, diapers, formula, etc. Consulted for cases of abuse, neglect, domestic violence, and sexual assault, provides counseling and connection with appropriate community support/resources and makes appropriate reports to state agencies. Completes on-going and timely documentation in the Electronic Medical Record.
MAJOR ACCOUNTABILITIES / CRITICAL RESPONSIBILITIES:
- Maintains a clinical caseload of patients.
- Participates in daily rounds.
- Maintains open communication with inter-disciplinary team.
- Completes psychosocial assessments and develops safe and appropriate discharge plans.
- Completes on-going screening for abuse, neglect and domestic violence for all ages and takes the appropriate action including reporting incidents to the proper authorities.
- Explores Advance Directives with patients, obtaining them if available, helping to complete if appropriate, and ensuring that once completed/obtained they become part of the Medical Record.
- Provides motivational counseling and develops interventions and/or makes referrals for alcohol/substance abuse cases.
- Insures timeliness of documentation of all interactions, interventions and outcomes. Completes required forms and assures that confidentially is maintained at all times.
- Participates in program development, performance improvement, and special projects and provides consultation to other departments and staff.
- Consults with Manager for problem solving and meets for supervision as scheduled.
- Maintains knowledge of current developments in the field through continuous training and education, and completes required CEU’s.
- Completes required continuous training and education, including department specific requirements.
- Demonstrates professional work ethic by following Service Standards and Success factors. Complies with departmental organizational policies and procedures and adheres to external agency requirements.
- Provides coverage for absent co-worker, when necessary.
- Provides weekend coverage as needed.
- Responsible for performing all other duties as assigned.
QUALIFICATIONS/REQUIREMENTS:
- LMSW required, LCSW preferred.
- Requires working knowledge of Medicaid and Medicare/health care financing.
- Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems.
- Work requires the ability to prepare detailed assessments of patient referrals, conduct individual or family meetings, or perform other duties requiring a comparable level of communication skill.
- Work requires ability to communicate orally and in writing.
- Work requires checking documents for accuracy.
