As our primary Social Worker supporting our residents and families you will: Complete clinical assessments and social service documentation as required by state / federal mandates and facility policy. Interviews residents and their family members to solicit information regarding the resident’s strengths, weaknesses, coping strategies and lifestyle prior to placement and develops and initial psychosocial assessment along with a detailed social history within the clinical record.
Develops and maintain resident psychosocial assessments and quarterly/routine care plans,
Educates residents, family members, and / or significant others regarding advance care planning and community resources
Works with the interdisciplinary team to assure resident and family concerns are addressed in a timely manner.
Advocates for the needs of residents in ensuring resident rights, educate residents, families, and staff on resident rights, resident abuse, mistreatment and neglect regulations to ensure residents are protected, provides information and documentation to residents and families with regards to access to Medicare, and financial and insurance benefits.
Collaborates with social service team members to ensure the needs of residents are met with professionalism, promote resident dignity, and resident choice.
Requirements Licensed Social Worker (BSN or MSN) required.
Degree in social work or related field. Minimum two years experience working in skilled nursing home required. (Long term care setting preferred) Must have strong clinical skills and demonstrate the ability to provide a safe and emotional support to residents of the facility.This Job Is Ideal for Someone Who Is:
