Outpatient Case Manager – Youngstown, OH
Salary $: Full-time; 40 hours per week position includes a competitive salary and benefits package.
Shift: Dayshift, M-F
Responsibilities:
- Needs assessment and formulation of the ISP. Individual therapeutic interactions/rapport-building: Crisis stabilization, ADL prompting, coaching and teaching. Client feedback
- Decisions pertaining to clients’ need for additional services, resources or more restrictive settings: Choice of treatment interventions, techniques, advocacy activity and frequency of contacts, Pursuit of collateral contacts to gather additional psychosocial information when indicated, Adherence to agency’s risk assessment/management protocols
- Intake/needs assessment, ISP and reviews, progress notes, Payeeship forms, discharge paperwork
- Conforms to Program, ODMH, CARF and third-party payer standards and obligations
- Interacting with essential others are constructive and serves to advance the progress in treatment, the quality of care and the agency’s standing in the community: Peers and supervisors, other service providers, family and significant other, other concerned members of the community
- Participates actively and productively in agency meetings and supervision sessions
- Introduces and/or implements new ideas for improving the quality of care
- Attentive/responsive to feedback from clients, significant others, community partners/members
- Shares skills, knowledge, expertise with other agency staff (consultations, workshops, team meeting)
- Advances own level of skill and competency through participation in continuing education opportunities
- Knowledge of community resources
Qualifications:
- High School diploma required. Bachelor’s degree preferred
- Previous mental health or community support/case management experience required.
- Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis.
- Knowledge of community resources is critical.
- CDCA is required if a candidate does not have a Bachelor’s degree; candidate will need to have prior to hire or will need to obtain within 45 days of hire.
- Experience using computers, Electronic Health Records (Credible), office phone and cell phone, email, fax/copy machine.

Miscellaneous requirements: Criminal background check; Pre-employment drug screen; Clear driving record; TB test and First Aid/CPR. Strong social, communication and written skills. Must have the ability to develop rapport and be perceived by clients and staff as approachable. Ability to multi-task; while maintaining accuracy and organization. Enthusiasm, initiative, self-motivated, organizational skills, ease in establishing rapport, ability to effectively communicate with the population, cultural sensitivity, willingness to provide services in the community including client homes
