ACT Nurse RN for Assertive Community Treatment

Job Overview

Join our dedicated Assertive Community Treatment (ACT) Team as a Nurse (RN), where you will play a pivotal role in delivering high-quality, compassionate care to individuals facing chronic mental health disorders. Key Responsibilities

• Conduct thorough psychiatric assessments and evaluate physical health needs.
• Make appropriate referrals to community physicians and manage medication administration in collaboration with the psychiatrist.
• Develop, implement, and revise personalized treatment plans and goals in conjunction with clients and the treatment team.
• Provide individual supportive therapy and symptom management, adjusting treatment plans based on evolving client needs.
• Educate and support clients’ families, advocating for their rights and preferences.
• Participate in comprehensive assessments covering a range of factors, including mental status and social functioning.
• Attend daily and weekly team meetings for organizational planning and supervision.
• Complete all necessary documentation in compliance with agency policies.
• Provide on-call crisis intervention as needed, and accompany clients to medical appointments to facilitate communication and understanding of treatment.
• Collaborate with the ACT team to address client health and medication concerns.

Qualifications

• An associate degree in nursing is required.
• A current RN license in Missouri is mandatory.
• Relevant experience in an inpatient or outpatient psychiatric setting is essential.
