Assertive Community Treatment Nurse RN/LPN

Job Overview

Compass Health Network is seeking a dedicated Nurse (RN/LPN) to join our Assertive Community Treatment (ACT) Team. In this critical role, you will be instrumental in managing our Medication Clinic, providing essential services to individuals with chronic mental health disorders. This position comes with a $2,000 sign-on bonus, reflecting our commitment to addressing the urgent needs of the communities we serve.

Key Responsibilities

• Organize and oversee the operations of the Medication Clinic.
• Act as a case manager for clients requiring medication refills.
• Conduct assessments of clients’ physical health needs and coordinate referrals to community physicians.
• Manage and administer medications in collaboration with the psychiatrist.
• Provide comprehensive treatment, rehabilitation, and support services to clients within the ACT framework.
• Maintain accurate documentation related to medication management and psychiatric consultations.
• Administer injections and oversee the medication dispensing process, including keeping precise logs.
• Facilitate the prior authorization process for medications.
• Monitor vital signs and assist in the pre-diabetes screening initiative.
• Engage in daily staff meetings and treatment planning sessions, and consult with the ACT team on health and medication matters.

• Proficient in utilizing computer systems for documentation and record-keeping.
• Strong organizational skills with the ability to coordinate diverse clinical activities.
• Capability to administer medications and manage medication systems effectively.
• Excellent communication skills for educating clients and collaborating with team members.

Qualifications

• High School diploma or GED is required; an Associate’s or Bachelor’s degree in nursing is preferred.
• Active RN or LPN licensure required; GPN candidates must pass the MO NCLEX exam within 90 days of graduation.
• Preferred experience includes working in an inpatient or outpatient psychiatric environment.
