You will lead team meetings and offer individual supervision that fosters both client and staff development, leveraging population health data to implement effective interventions, and educate both clients and staff in collaboration with the Physician Consultant. Key Responsibilities
- Develop and maintain collaborative relationships with primary and specialty care providers, as well as inpatient facilities.
- Create models and processes to foster integrated treatment across all services.
- Track enrollment, declines, discharges, and transfers, ensuring optimal participation in the program.
- Facilitate the development of partnerships (including Memoranda of Understanding) with hospitals and coordinate admissions and discharges with Nurse Care Managers.
- Oversee daily operations of the Healthcare Home and implement activities that enhance service delivery.
- Monitor performance metrics and lead quality improvement initiatives.
- Manage and support a team of Nurse Care Managers, providing both administrative and clinical supervision.
- Ensure adherence to DMH regulations and service provision expectations while conducting regular supervision and team meetings.
- Work collaboratively as part of a multidisciplinary team to provide holistic care for clients.
- Strong leadership and administrative capabilities.
- Excellent communication and interpersonal skills to build relationships with healthcare providers and clients.
- Knowledge of health regulations and compliance requirements.
- Proficiency in care management software and electronic medical records (EMR) systems.
- Strong organizational and problem-solving skills.
- Associate’s or Bachelor’s Degree in Nursing.
- Minimum of two (2) years of experience working with the chronically mentally ill population is required, with supervisory experience preferred.
- Relevant experience in the healthcare field is favorable.
- Must be licensed by the State Board of Nursing as a Registered Nurse.
