Key Responsibilities
- Provide supervisory oversight and training for Nurse Care Managers.
- Maintain a therapeutic environment compliant with relevant standards and regulations.
- Lead team meetings and individual supervision sessions to foster professional development.
- Utilize population health data, with input from the Physician Consultant, to guide interventions and ongoing staff training.
- Collaborate in quality improvement initiatives and uphold documentation standards.
- Develop and sustain relationships with primary and specialty care providers.
- Oversee daily operations of the Healthcare Home, ensuring optimal enrollment and service delivery.
- Champion the transformation of healthcare practices within the organization.
