Key Responsibilities
- Coordinate patient appointments and arrange transportation resources.
- Collaborate with the Primary Care team to identify barriers to self-management observed during patient visits.
- Promote health and wellness by delivering culturally relevant clinical health information and education to patients.
- Support patients in taking proactive steps toward managing their conditions and improving their health.
- Enhance patients’ understanding of their health conditions and the available resources.
- Assist in identifying and securing clinical, social, and community services.
- Follow up with patients post-hospitalization or visits to ensure continuity of care.
- Engage with patients, caregivers, and families to support the achievement of health goals respecting cultural practices.
- Maintain regular communication with patients in person and via phone.
- Document patient interactions and care plans accurately in electronic health records.
