Job Overview

The Community Health Worker - LPN serves as a vital clinical connector between health systems and social services, striving to enhance access to essential resources while improving the quality and cultural competence of care delivery. This role is instrumental in building both individual and community capacity through enhanced health knowledge and self-sufficiency. Activities include clinical evaluations, outreach initiatives, community education, informal counseling, social support, and advocacy. By linking health systems to local community resources, this position aims to elevate the health outcomes of Missourians through clinical education focused on mitigating behavioral health risks.

Key Responsibilities

  • Coordinate patient appointments and arrange transportation resources.
  • Collaborate with the Primary Care team to identify barriers to self-management observed during patient visits.
  • Promote health and wellness by delivering culturally relevant clinical health information and education to patients.
  • Support patients in taking proactive steps toward managing their conditions and improving their health.
  • Enhance patients’ understanding of their health conditions and the available resources.
  • Assist in identifying and securing clinical, social, and community services.
  • Follow up with patients post-hospitalization or visits to ensure continuity of care.
  • Engage with patients, caregivers, and families to support the achievement of health goals respecting cultural practices.
  • Maintain regular communication with patients in person and via phone.
  • Document patient interactions and care plans accurately in electronic health records.
  • Adhere to care coordination requirements as per state health initiatives.
  • Foster collaborative relationships with community health and social service providers.
  • Participate actively in primary care team meetings and training sessions.
  • Demonstrate professional respect and responsibility towards patients, families, and colleagues.

Qualifications

  • Completion of a practical nursing program is required.
  • High School diploma or GED is required.
  • LPN licensure in Missouri is mandatory.
  • Successful completion of an accredited Community Health Worker program is essential.
